Pharmacy teams are a trusted resource for patients navigating chronic kidney disease. Recent guideline updates note important changes to treatment of which practitioners must be aware to best support their patients in the management of this disease. This course will provide updates on the newest recommendations for kidney function estimation and treatment of hypertension and type 2 diabetes in patients with chronic kidney disease. This course will equip the pharmacist learner with the knowledge and skills required to provide care and management for this population.

  2. Assign a goal blood pressure for a patient with chronic kidney disease.
  3. Optimize antihypertensive therapies in a patient with chronic kidney disease to achieve improved clinical outcomes.
  4. Assign a hemoglobin A1C target for a patient with chronic kidney disease and type 2 diabetes.
  5. Optimize medication therapy in a patient with chronic kidney disease and diabetes to achieve improved clinical outcomes.
