Many entities are looking for innovative ways to address Social Determinants of Health (SDoH) issues that are negatively impacting patient outcomes. Interventions must be local and meaningful to the patients engaged, but most importantly, these interventions must be longitudinal. Community pharmacy teams are easily accessible, possess strong, established relationships with the community members they serve, and are equipped to provide medication-related education and services. In short, community pharmacy teams are the perfect innovative solution to solving SDoH-related issues.
