Patients transition from various care settings every day. Collaboration among providers from different settings can prevent errors and improve overall care.  Pharmacists in various practice settings can offer valuable insights into potential risks as patients transition from one care setting to another. This course will highlight the transitions of care process and the potential risks that can be encountered. Learners will be inspired to look for collaborative opportunities to optimize patient care.

Upon successful completion of this application-based CPE activity, participants should be able to:

  1. Describe how pharmacists interact with and care for patients moving through different care settings.  
  2. Discuss an example of a prescribing process, developed through the collaboration of pharmacists, for a patient experiencing a care transition.
  3. Explain the importance of meeting the educational needs of patients during a transitional process.
  4. Identify opportunities to optimize outcomes during transitions of care. 
  5. Propose methods to improve patient care through collaboration.
