Pharmacy teams are a trusted resource for patients on anticoagulation therapy. Recent guideline and literature updates related to
perioperative management, use of direct oral anticoagulants (DOACs) in obesity, and data showing concerns for DOACs in certain conditions will be discussed. This course will equip pharmacy professionals to support their teams and patients in anticoagulation management.

Learning Objectives - Pharmacists
Upon successful completion of this application-based CPE activity, participants should be able to:

  1. Review key updates on perioperative anticoagulation management from the 2022 CHEST Guidelines.
  2. Differentiate when to recommend perioperative anticoagulation bridging for patients taking warfarin for various indications.
  3. Discuss the guidelines and current recommendations for the use of direct oral anticoagulants (DOACs) in obese patients.
  4. Analyze the literature related to potential setbacks when using DOACs in patients with certain conditions.
  5. Compile a list of key points to use when implementing these updates into anticoagulation management at a practice site.
