Medically complex patients pose unique challenges. Lack of access to transportation can add an additional barrier to care. This session will highlight the importance of pharmaceutical care for medically complex patients in rural areas or with transportation issues. These factors have a direct impact on overall patient health, and pharmacists can improve outcomes by developing care plans that address these barriers.
